New business makes health-care transitions easier

Transition Home gives patients short-term care to work out transitions

Moving from hospital care to home care, or from the home to a long-term care setting can be a stressful and difficult time for patients and their families.

To make those transitions a bit easier, Kimberly Wagg, a registered practical nurse with 30 years experience, has launched a new service called Transition House

Wagg has access to four two-bedroom suites in the Regency Park apartment building she uses to help patients as they transition from one type of care to another.

“Because I've worked actively in nursing in the hospital and in the community there seemed to be a hole that needed to be filled,” she said. “Where do you go for short-term stay?”

If a person is discharged from hospital, but has limited mobility, for example, they may not be ready to move back into their home right away. 

Through Transition House they can stay at a fully accessible suite, with access to 24-hour attendant care services from personal support workers, while they get their affairs in order.

Wagg said most of her clients end up staying at the suites between three and 10 days before they are ready to move on to care at their homes or in another setting.

In addition to access to personal support workers, Transition Home also has massage and foot care services available. 

Clients must pay out of pocket to access the service, but private insurance plans might cover some of the costs.

Prices are based on the level of care a patient requires.

Wagg said she is currently in talks with the North East Local Health Integration Network to help subsidize some of the costs.

“I don't think you should be penalized for getting sick,” she said.
