The Independence Centre and Network's Post Stroke Transitional Care Program made the Ontario health minister's team-based honour roll for excellence in health quality and safety on Oct. 20.

“The fact that we're getting recognized at a provincial level is really nice,” said Rebecca Bowes, regional co-ordinator for the program, which helps people across northeastern Ontario recover after a stroke.

Bowes and her colleagues received the honour at the Health Quality Ontario Transformation Conference in Toronto.

The Post Stroke Transitional Care Program started in Sudbury in 2011, and expanded across northeastern Ontario in 2014.

In the 2015/2016 fiscal year more than 380 clients received more than 5,800 rehabilitative sessions through the program.

Bowes said all clients surveyed have reported an improvement to their health since participating in the program, and 82 per cent have reported making positive lifestyle changes since they got involved.
