North East CCAC recognized for e-Notification system

The North East Community Care Access Centre's (CCAC) e-Notification information-sharing project won the System Partnership Award at the Ontario Association of Community Care Access Centres' Awards for Excellence Banquet in Toronto May 28.

The awards were part of the 2015 Conference of the Ontario Association of Community Care Access Centres.

The e-Notification software relays patients' names and birth dates to a provincial database when they are admitted to the emergency department or a hospital inpatient unit.

The information is relayed to a provincial database, and if the patient is also a North East CCAC patient, both the hospital and CCAC systems are notified.

Funded by eHealth Ontario, the new North East CCAC-led e-Notification information-sharing project has been in full operation at 24 Northeastern Ontario Network hospitals since March 31.

“This project has seen an unprecedented level of cooperation among partners for the benefit of our patients,” said Peter Taylor, e-Notification steering committee chair, and North East CCAC director of business solutions.

“E-Notification enables patients to experience smoother transitions, faster discharges, fewer readmissions to hospital, and better follow-up care.”

As System Partnership winners, selected team members will also be invited to present at the Canadian Home Care Association’s International Home Care Summit in Ottawa in November, profiling the e-Notification project to delegates from across Canada and around the globe, and being featured in a special CHCA report to be shared Canada-wide.
